The following documentation will be requested in order to meet all eligibility requirements:
- Copy of valid government-issued ID from all signers
- Copy of Articles of Organization/Incorporation, or Fictitious Name Filing, etc., based on business registration
- IRS letter reflecting the business name and federal tax identification number
- The business must be listed as "active" on Sunbiz.org and must reflect tax identification number on the site
